If you have the original receipt:

1. Fill out the tracking form on the back of the receipt.

2. Submit the receipt along with a $15.00 fee.

The address is listed on the receipt.

If you do not have the original receipt:

1. Go to the WU website and print the customer request form.

2. Submit this form along with a $30 fee.

The website offers three methods to submit the customer request form.
